Parental consent for child to travel

Do you need to sign a consent form allowing your child to travel overseas?

What you need to do:

Book an appointment with us, and bring along your 'Consent to travel' form (this form may have different titles).

Both parents will need to attend the appointment, and they will need to bring along 2 forms of photo identification (preferably your passports and drivers licences) as well as the birth certificate for the child.

Once I have checked the documents and verified all identities, I will prepare my notarial certificate certifying the execution of the travel authorisation documents.  The process takes less than 30 mins.
